CSG online enrolment form
This form is to apply for a place in the class. You will receive an invoice upon being put into the class and your place is confirmed upon payment. To enrol many attendees, complete this form once for each attendee.

Please note, Due
to circumstances beyond our control E-Mails being sent to Gmail and some
Hotmail accounts are no longer being delivered to your Inbox, but instead being
directed to Spam or Junk mail boxes. Please check these folders regularly for
communication from CSG. Once you have received an E-mail from us, please move
the E-mail into your inbox and/or mark as a safe sender, this should direct
future E-mails into your inbox. We endevour to contact all new enrollment by phone, but this cannot be guaranteed as our prime method of communication is Via E-mail.

The information contained within this form will be used primarily for matters specifically related to participating in gymnastics and/or if a secondary purpose is related to the primary purpose and one could reasonably expect such use or disclosure.
To assist in providing our services, the organisations to which we disclose information include :-
NZ Gymnastics, Canterbury Gymnastics, Our Insurers, The Sport Education Sector, our accountants, lawyers auditors, Government and regulatory authorities and other organisations, as required or authorised by law. We limit the use and disclosure of any personal information provided by us to provide you with the services you require, or the level of service on which we pride ourselves on. If you chose not to provide personal information, we may not be able to provide you with the services you require, or the level of service on which we pride ourselves.
Participation in Gymnastics activities carries with it a reasonable assumption of risk. Please note that this information is not intended to constitute legal advice, and is provided as general information only. You should not rely on it without first verifying the accuracy, completeness and currency of the material, and it's relevance to your individual circumstances.

participating in any club activities and consent to these being used for

7. deposits and Fees are non-refundable. There are no
refunds/credits if you withdraw from or miss classes. Refunds only apply if we
receive a minimum of 3 working days notice prior to the first class of term – a $15
administration fee will apply for full fee refunds and a $5 dollar fee will apply for deposit refunds.
